1) Filling History Form
- For taking Homoeopathic treatment from Dr. Deepa Karia, it is necessary for you to submit a duly filled/completed History Form. The history form is a very detailed document containing information about your disease/complaints as well as details about yourself as a person. This form is designed to obtain the information required for reaching a correct remedy for you.
- History forms for Adults and History forms for Children are different. Forms are available in four different languages (4 languages – English, Gujarati, Hindi, and Marathi). Select the form which you want to use from below.
- The form can be filled by using plain paper sheets by hand. Once completed, you can scan it and send it to us by email to care@drdeepa.in or WhatsApp/Telegram to +91-9223355109/+91-7045294148.
- If you wish to type your answers, you could download the form in MS-Word format and send the completed form back to us by email to care@drdeepa.in or WhatsApp/Telegram to +91-9223355109/+91-7045294148.
- You can also choose to fill the form online on this website and submit it online (English Adult History Form, English Child History Form).
- You also need to send electronic copy of past investigation reports along with the form.
- Once your form is submitted you would be given an appointment at mutual convenience. Appointment can also be obtained on our phone numbers (+91-22-25602669/70) or by sending WhatsApp/Telegram message on +91-9223355109/+91-7045294148.

2) Consultation via Telephone + Video Call
Once you submit your form as explained above, you will be given an appointment for first consultation via telephonic consultation. The conversation will be in two parts – telephonic and video call. Usually, the first consultation takes over an hour or so. Please plan your first consultation accordingly.
3) Dispensing the Medicine
You will be given medicine after your first consultation which can be sent via courier (at your cost) or you could have it collected from the clinic. You will be explained the manner of taking the medicine and the care to be taken (Click here for more details) when consuming the medicine.
4) Follow up via Telephone
A follow up meeting or call will be scheduled at mutual convenience for assessing the impact of medication. Follow up usually doesn’t take too much time unless there is a need for spending more time in certain situations. After follow up consultation, necessary medicine will be sent via courier (at your cost) or you could have it collected from the clinic. This process will continue at a certain periodicity till the disease is completely cured.
